Job Title: Certified Nursing Assistant (CNA) – Long Term Care

Pay Range: $23/hour (W2)
Location: Brooklyn, NY 11212
Shift: Evening (3:00 PM–11:00 PM) & Night (11:00 PM–7:00 AM) | 5 Days/Week | Every Other Weekend REQUIRED | NO RTO | Must reside within 50 miles
Required Education & Certifications (Mandatory)
• High School Diploma or GED (REQUIRED)
• Completion of at least 100 hours from a New York State approved Nursing Assistant training program (REQUIRED)
• Active New York Nurse Aide Certification AND listing in NY State RHCF Nurse Aide Registry (REQUIRED)
Required Skills & Experience (Mandatory)
• Active NY Nurse Aide Certification listed in NY State RHCF Registry (REQUIRED)
• Experience providing hands-on patient/resident care in Long-Term Care setting (REQUIRED)
• Ability to assist with ADLs (bathing, grooming, feeding, toileting, hygiene, mobility support) (REQUIRED)
• Knowledge of skin breakdown prevention and patient safety practices (REQUIRED)
• Ability to take and record vital signs (BP, pulse, respiration, temperature, weight) in EMR (REQUIRED)
• Ability to follow verbal and written instructions accurately (REQUIRED)
• Strong compassionate communication skills with elderly/cognitively impaired residents (REQUIRED)
• Ability to work physically demanding tasks (lifting, turning, transferring patients) (REQUIRED)
Preferred Skills & Experience
• Long-Term Care (LTC) experience
• Experience with mechanical lifts and patient transfer assistance
• Experience in high-acuity or dementia care settings
Job Description / Project
The Certified Nursing Assistant will provide direct, hands-on care to residents in a Long-Term Care facility, ensuring safety, comfort, and quality of life.
Key responsibilities include:
• Assisting residents with activities of daily living (ADLs) including bathing, grooming, dressing, toileting, and oral care
• Supporting mobility, transfers, ambulation, repositioning, and fall prevention
• Monitoring and recording vital signs, intake/output, weights, and behavioral changes
• Feeding and assisting residents with meals and hydration needs
• Providing care for cognitively impaired, terminally ill, or high-dependency residents
• Maintaining clean, safe, and organized resident environments
• Documenting care in EMR and reporting changes to nursing staff
• Responding promptly to call bells, alarms, and emergency situations
• Assisting with admissions, discharges, and resident transport
